- Fill in the Contact Information such as First Name, Last Name, Title, and Direct Email Address. This will also be used as your web login ID.
- Fill in the Business Information about your business, primary interest, how you heard about them, number of employees in your location, and number of outside sales reps.
- An account will be created with the login ID using the Direct Email Address. The Password must be between 8 and 20 characters long, must begin with a letter and cannot contain any 3 consecutive characters from your login ID.
- Confirm the password and enter a password hint to help you remember your password if you forget.

Please note that all returns must be reported within ten (10) days from receipt of merchandise. A written authorization number must be obtained for all returns.
